PACE3: Influence Care Delivered in the Home

Welcome back to our PACE3 email series. In this installment we talk about one of the major issues facing PACE today, the lack of insight into what's happening at home. When participants are in our centers we know exactly what's happening. But when they leave, it's a mystery what happens in the home. Wouldn't it be great if you could influence what happens in those hours when participants are out of your sight?

Here's what we're hearing from you.

"We have no idea what happens when the participant leaves the center." PACE RN

"Half of our participants don't even come into the center. We need them to come in." PACE Clinical Supervisor

Today, current solutions include sending paper instructions home with the participant for the family caregiver. For example, some PACE organizations send home a paper calendar, which shows what care tasks should be completed when participants are home. Paper-based solutions depend on visual availability in the home, as well as the memory of the participant's family caregiver in the time of need. Further, these solutions offer no feedback for the IDT. There's no way to know if care tasks are being done.

At Care3, we agree that the lack of insight into care being delivered in the home is one of the top risks to your PACE program success. We believe care delivered in the home is just as important as care delivered in the center, so actions for BOTH settings should be included and tracked in care plans.

Care3 can help you influence all of the care delivered in the home by engaging the family members with specific actions in the care plan. Each action is itemized for the caregiver to complete in the home and then is tracked to ensure that those care tasks are completed. If those care tasks are not completed, Care3 notifies the IDT so that they can make a decision whether to intervene. This is what makes Care3 different from every other care planning and communication solution available to PACE today.
